Insurance

We accept most insurance plans. Please check with your insurance provider or our office to obtain specific coverage information. Please check your insurance provider to determine the procedures you need to follow in order to have your gastroenterology treatment covered. For example, you may need a referral from a primary care physician to be treated by a specialist. If you do, please obtain the referral prior to your visit and bring the referral with you. You may not be covered for your treatment if you do not follow the procedures required by your insurance carrier. This may result in being responsible for the cost of your treatment.

Payments

Payments and co-payments are due at the time of your visit.

In an effort to facilitate your visit, you can print and complete the following forms before your visit and bring them with you.

General Instructions

On the day of your procedure you should be dressed in loose comfortable clothing. If you are having an upper endoscopy, please wear a short sleeve shirt. We suggest that you leave all personal and valuable items at home for the day. You must have a driver as you will be under anesthesia and unable to drive for 24 hours. We anticipate that you and your driver will be at the office for approximately 2 hours.
